These Terms of Service govern your use of the services offered by Revitalize Your Glow MedSpa, operated by OLA RLT LLC (“Practice”, “we”, “us”, or “our”). By scheduling an appointment, submitting a booking request, or using our services, you agree to the following terms.

1. Appointments

We recommend scheduling your appointment in advance to ensure availability at your preferred location. Certain treatments may require a deposit or prepayment at the time of booking.

The Practice reserves the right to collect a non-refundable deposit for any appointment. This deposit will remain as a credit on your account and may be applied toward your treatment or future services.

2. Booking & Cancellation Policy

Appointments may be booked online, by phone, or in person. A credit card may be required to secure your booking.

We require 24-hour notice for cancellations or rescheduling. No-shows and last-minute cancellations may incur a cancellation fee of up to 50% of the service price. Repeat no-shows may prevent future bookings.

3. Payments & Refunds

Full payment is required before or at the time of service. We accept Visa, MasterCard, American Express, Discover, and digital payment methods. Prices are subject to change without notice.

• Services: All service sales are final. No refunds will be issued.
• Products: Unused, unopened products may be returned within 14 days of purchase for store credit.
• Final sale items: Digital gift cards, travel-sized items, and promotional bundles are non-refundable.
• Faulty items: If you receive a defective product, contact us within 7 days for a possible exchange.

4. Eligibility

You must be at least 18 years of age to use our services. By scheduling or receiving services, you represent that you have the legal capacity to enter into a binding agreement. If using services on behalf of another, you confirm their consent.

5. Medical Disclaimer

Our services are not a substitute for professional medical advice. Consult your healthcare provider before treatments if you have any medical conditions. We are not liable for adverse reactions from treatments.

6. Liability & Indemnification

Revitalize Your Glow is not responsible for any injuries, allergic reactions, or adverse effects resulting from our services or products. By using our services, you waive any claims against Revitalize Your Glow, its employees, and affiliates. You indemnify and hold the business harmless from legal claims arising from service use.

7. SMS / Text Messaging Program

The sole point at which you opt in to receive SMS messages from us is the contact form on our /contact page. The form presents two independent consent checkboxes — one for transactional (non-marketing) messages and one for marketing messages. Both checkboxes are unchecked by default, neither is required to submit the form, and you may check one, both, or neither.

How to Opt Out: To stop receiving SMS messages at any time, reply STOP to any text message from us. You will receive a final confirmation and no further messages will be sent unless you re-subscribe.

Carrier Liability Disclaimer: Mobile carriers are not liable for delayed or undelivered messages. Delivery of SMS messages is subject to effective transmission from your network operator. Standard message and data rates may apply per your carrier plan.

8. Binding Arbitration

Disputes related to these Terms or our services will be resolved through binding arbitration rather than in court. By agreeing to arbitration, you waive your right to a jury trial or participation in class-action lawsuits. Arbitration is conducted under the American Arbitration Association (AAA) rules.

These Terms are governed by the laws of the State of Arizona, without regard to conflict of law principles.

We reserve the right to update these Terms at any time. Updates become effective immediately upon posting. Continued use of our services constitutes acceptance of the revised Terms.
